Alteration Permit Application:  When an alteration to an existing elevator, escalator, platform lifts, moving sidewalks, vertical conveyors, stairway lifts and wheelchair lifts is proposed. You will be mailed the permit upon the processing of the application in approximately seven business days. NOTE: All applications shall be in duplicate, covering one device only, and shall be accompanied by plans and specifications in duplicate. Payment in check or money order shall accompany application.

Construction and Installation Permit Application:  When a new elevator, escalator, platform lift, moving sidewalk, vertical conveyors, stairway lift and wheelchair lift is to be installed.  You will receive a response to the plan review in approximately seven business days.  NOTE:  All applications shall be in duplicate, covering one device only, and shall be accompanied by plans and specifications in duplicate.  Payment in check or money order shall accompany application.
 

Elevator Continuing Education Provider Application: Utilized by companies or individuals who want to provide continuing education to the elevator industry. A form is required for each class to be provided. All classes that are approved for continuing education will be posted under our "Education" tab located at the top of this page.

Elevator Continuing Education Course Application: Submitted by elevator contractors or mechanics seeking their required annual continuing education credits.

Elevator Contractor License Application: New elevator contractors who desire to enroll in the contractor's license program to be approved to install or alter elevator and other applicable equipment in Kentucky. Each elevator contractor/company shall employ a minimum of one Kentucky licensed elevator mechanic.

Elevator Mechanic or Accessibility and Residential Elevator Mechanic License Applications:  New individual elevator mechanics who desire to enroll in the mechanic's license program to be approved to install or alter elevator and other applicable equipment in Kentucky; or accessibility and residential mechanic who desires to enroll as a limited mechanic and be approved to properly install, maintain, repair, remodel or alter accessibility lifts and private residential elevators. 

Elevator Renewal Application:  Renewal application for elevator mechanics and contractors.

Safety Test Form:  A guideline on the items that are inspected on the one- and five-year elevator, escalator and walkway inspections.

Fee Schedule: A tool to calculate the required fees for new or altered elevator work including all other applicable equipment. All fees are made payable to the Kentucky State Treasurer.
